Community Health Worker - Enhanced Care Management (Finance)
Position Summary:
A Community Health Worker (CHW) focuses on promoting community-based health education and prevention in underserved communities. The Community Health Worker serves as the bridge between community residents and health care institutions and professional providers with a focus on preventive health services to prevent disease, disability and other health conditions or their progression; to prolong life; and promote physical and mental health and well-being.
Qualifications:
1. High School diploma or GED required.
2. Proficient in Microsoft suite (Word, Excel, PowerPoint).
3. Strong computer skills/data entry.
4. Knowledge of community resources.
5. Bilingual in Spanish preferred.
6. A valid California drivers license is required of staff who have regular driving duties and the driving record must meet the requirements of their position and be approved by the agencys insurance company.
Essential Job Functions:
1. Physical capability to lift over 20 pounds, and engage in physical activity including sitting, walking, bending, climbing, kneeling, twisting, reaching, etc.
2. Ability to communicate ideas in oral and written form.
3. Ability to instruct in group and individual settings.
4. Physical, mental, and emotional health adequate to perform duties.
5. Ability to work effectively with cultural/ethnic diversity.

7. Ability wo work in a harmonious manner with subordinates, supervisors, and others.
8. Adhere to all applicable legal and ethical standards.
Responsibilities:
1. Utilize prevention strategies to promote stigma and discrimination reduction, suicide prevention, mental health awareness and well-being.
2. Provide access, advocacy, resources and linkage to clients and their families.
3. Interact productively within a multi-disciplinary team and other community organizations.
4. Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
5. Responsible for preventing and controlling infection.
6. Responsible for maintaining a culture of quality and safety.
7. Effectively use de-escalation skills and techniques for crisis prevention.
8. Properly administer the C-SSRS to help prevent suicide.
