Key Responsibilities:

• Evaluate patients and develop individualized treatment plans to improve physical function and mobility
• Provide therapeutic exercises, manual therapy, and education to patients
• Work closely with an interdisciplinary team to ensure comprehensive patient care
• Monitor progress and adjust treatment plans as necessary
• Maintain accurate and timely patient documentation

Qualifications:

• Valid CA Physical Therapist license
• Experience in a skilled nursing or similar healthcare setting preferred
• Strong communication and collaboration skills
• Compassionate and patient-focused
