RN (Personal Services)
Registered Nurse (RN) with an unencumbered valid NH nursing license.
CPR / BLS certification required (ACLS and other certifications may be required upon employment).
Experience in primary care, public health, or community health settings preferred.
Knowledge of common lab values and their significance.
Demonstrate understanding of commonly prescribed medications and treatments.
Knowledge of chronic disease management, preventive health services, and patient care techniques.
Familiarity with Electronic Health Records (Centricity experience preferred).
Ability to work in a collaborative, team-oriented environment.
Strong communication and interpersonal skills, with a focus on cultural competence.

Preferred Qualifications:
Bachelor's degree is preferred, but not required
Demonstrated resourcefulness, flexibility and integrity.
Experience working in a Federally Qualified Healthcare Center (FQHC).
Experience in a community healthcare setting.
Experience with substance abuse and Medication assisted treatment practices.
Knowledge of Microsoft Office and Centricity EMR software preferred.
Bilingual (Spanish/English) is preferred, but not required.
Knowledge and Abilities:
Excellent oral, written and reading communication skills.
Ability to understand and execute verbal and written instructions.
Ahere to HIPAA rules and regulations by actively participating in safety measures for safeguarding patient information confidentiality.

Responsibilities:
Advocate for patients with health concerns by using a systematic approach to history and physical appraisal of patients and/or families, through clinical judgment of evaluated data.
Respectfully communicate patient issues and concerns with providers.
Medication IM and SC administration and education as needed.
Perform blood draws and laboratory processing as necessary.
Monitor patients with long term or chronic illness and supports individuals and their families.
Plan appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and agencies.
Instruct individual and/or families or groups about health or defined health problems.
Participate in Harbor Care Quality Assurance activities.
Participate in education of nursing students as assigned.
Serve as a clinical resource for CMA's.
Promote community relations; anticipates and responds to the needs of others in a courteous, efficient manner, while maintaining patient privacy and confidentiality.
Maintain a neat, clean, professional workspace.
Adhere to Harbor Care dress code (Scrubs and appropriate footwear).
Complete all necessary paperwork and reporting in a timely manner.
Participate in monitoring and maintaining vaccine supply along with ensuring State and Federal reporting requirements.
Participate in providing training throughout Harbor Care to staff, residents, and patients.
Attend interdisciplinary meetings as appropriate.

Complete other tasks assigned by Nurse Manager.
